The purpose of wraparound is to establish one working plan even if the child is receiving services from other agencies in addition to the services provided by Hathaway-Sycamores Child and Family Services. A child qualifies for the program if he or she is eligible to receive services from two or more of the following Los Angeles County departments and public agencies: Department of Children and Family Services, Department of Mental Health, Probation, special education programs or if the child is at risk of being placed in a group care home RCL 12 or 14 or has a history of multiple placements.
Connections offers children and families team networking and resource development, one-to-one support for parents and children, crisis intervention, individual and family clinical services, educational support services and integrated planning for long-term community resources. Staff will provide progress reports to the court if necessary; individuals receive Certificates of Completion at the end of the program.
The self-injury program provides counseling and family support for children, adolescents and adults who are engaged in a cycle of self abuse, including cutting. Wraparound is conducted by a multi-displinary team consisting of a social worker case manager, a therapist and a parent "Partner". Sessions are conducted in the home and may involve just the child or the whole family as needed.
The program accepts youth from age 8 to 18, and may continue for one year or longer. Call Us Wraparound is a family-centered, strengths-based, needs-driven planning process. Wraparound supports family voice, choice and ownership of strategies to return or maintain children in their community with normalized and inclusive community options, activities and opportunities.
Wraparound requires perseverance to create and provide a highly individualized planning process and to continue until the desirable outcomes for the child ren and families are achieved. Youth referred for wraparound services are those identified as highest needs children in Los Angeles County. Wraparound provides the family with support services which can range from 7 days a week up to 24 hours a day to once a week, ranging from crisis intervention and advocating for family needs within the Los Angeles Department of Children and Family Services and Los Angeles County Probation Department.
The average length of services for families range from months. Wraparound consist of three phases which include the Initial plan development phase, Implementation phase and transition phase.
